Hardcopy Applicaton to join IPMS?

I'm curious if there is a application form that can be printed out for joining IPMS USA. If so, where is it located?

But I recommend you go to the main page, select the menu button "Join IPMS/USA" and look at the pricing, the mailing address,etc -- and consider the online application through the Store -- it works great.
